To get into a Japanese speaking University, yes you do. For even some language schools just to learn Japanese you need a certain level of JLPT or have studied a certain amount of hours of Japanese before you can enroll (example 300 hours of learning time), not all these language schools require prior learning but you would be suprised by the amount of them that do. The same applies to Universities in my home country, you need to have a satisfactory level of English before you can enroll.
There are some (not a huge amount) English teaching Universities in Japan but they are mostly for Postgraduates looking to get their masters or PHD. Not too mention they are very expensive courses to do (for both Undergraduates and Post Graduates)
